Clinical Overview
Pyridostigmine Bromide is a reversible carbamate quaternary ammonium acetylcholinesterase inhibitor on the WHO Model List of Essential Medicines. It inhibits the enzymatic degradation of acetylcholine at the neuromuscular junction, prolonging and amplifying nicotinic receptor activation. Its quaternary structure prevents blood-brain barrier penetration, focusing action on peripheral muscles without central toxicity. Indicated for the symptomatic treatment of myasthenia gravis and reversal of non-depolarizing neuromuscular block.

Usage Guidelines
Take orally with water and food or milk to minimize gastrointestinal cramps. Time doses carefully prior to anticipated physical exertion or meals (typically 30-45 minutes before meals) to aid chewing and swallowing. Follow strict dosing intervals.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Diarrhea, nausea, abdominal cramps, increased salivation, muscle twitching (fasciculations), diaphoresis, bronchial hypersecretion. Rare/Severe: Cholinergic crisis (excessive acetylcholine causing severe muscle weakness, bronchospasm, bradycardia, respiratory arrest), cardiac arrhythmias.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Critical distinction: Must distinguish between myasthenic crisis (under-treatment) and cholinergic crisis (over-treatment), as both present with extreme muscle weakness; increasing pyridostigmine in a cholinergic crisis can be fatal. Contraindicated in mechanical urinary tract or intestinal obstruction.
