Clinical Overview
Tacrolimus Capsules (0.25mg, 1mg, 2mg) is a potent macrolide calcineurin inhibitor on the WHO Model List of Essential Medicines. It binds to intracellular FK506-binding protein-12 (FKBP-12); the drug-protein complex binds and inhibits calcineurin phosphatase with 10- to 100-fold higher potency than cyclosporine, extinguishing IL-2 gene transcription, T-cell proliferation, and cytotoxic lymphocyte generation. Indicated for prophylaxis of organ rejection in patients receiving allogeneic kidney, liver, or heart transplants, and rescue therapy in refractory rejection.

Usage Guidelines
Take orally in two divided doses every 12 hours on a consistent schedule on an EMPTY STOMACH (at least 1 hour before or 2 hours after meals with water), as food significantly reduces absorption. Therapeutic Drug Monitoring (TDM) of whole blood trough levels (C0) using specific immunoassay is MANDATORY to maintain blood concentrations within target ranges (typically 5-15 ng/mL).
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Tremors, headache, hypertension, nephrotoxicity (elevated creatinine), new-onset diabetes after transplantation (NODAT / post-transplant diabetes mellitus due to pancreatic islet cell toxicity), hyperkalemia, hypomagnesemia, diarrhea. Rare/Severe: Posterior reversible encephalopathy syndrome (PRES), severe opportunistic infections, and post-transplant lymphoproliferative disorder (PTLD).
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Susceptibility to fatal infections and malignancy (lymphoma, skin cancer). Black Box Warning: Nephrotoxicity and neurotoxicity; mandatory whole blood trough level monitoring. More diabetogenic than cyclosporine but does NOT cause cosmetic hirsutism or gingival hyperplasia. Strictly avoid grapefruit juice and St. John's wort.
