Tacrolimus

Tacrolimus (0.03%, 0.1% ointment) is a topical calcineurin inhibitor (TCI) and non-steroidal immunosuppressant on the WHO Model List of Essential Medicines. It binds to intracellular immunophilin FKBP-12, forming a complex that inhibits calcineurin phosphatase, preventing dephosphorylation of NF-AT and blocking transcription of pro-inflammatory cytokines (IL-2, IL-3, IL-4, IFN-gamma) in T-lymphocytes. Unlike steroids, it does not cause skin atrophy, telangiectasia, or collagen breakdown. Indicated for moderate-to-severe atopic dermatitis (eczema) in patients not adequately responsive to or intolerant of conventional topical therapies, especially on the face, neck, and flexures.

It binds to intracellular FK506-binding protein-12 (FKBP-12); the drug-protein complex binds and inhibits calcineurin phosphatase with 10- to 100-fold higher potency than cyclosporine, extinguishing IL-2 gene transcription, T-cell proliferation, and cytotoxic lymphocyte generation. Indicated for prophylaxis of organ rejection in patients receiving allogeneic kidney, liver, or heart transplants, and rescue therapy in refractory rejection.
