Clinical Overview
Ticagrelor is a direct-acting, reversibly-binding cyclopentyltriazolopyrimidine P2Y12 platelet antagonist on the WHO Model List of Essential Medicines. Unlike thienopyridines (clopidogrel, prasugrel), it does not require metabolic bioactivation, offering rapid, predictable, and reversible platelet inhibition. Indicated for the prevention of atherothrombotic events in adult patients with acute coronary syndromes (ACS) or a history of myocardial infarction and high risk of developing an atherothrombotic event.

Usage Guidelines
Take orally twice daily (every 12 hours) with water, with or without food. Co-administered with low-dose aspirin (75-100 mg daily). Do not miss doses due to reversible binding.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Dyspnea (shortness of breath, usually self-limiting), bleeding, bruising, epistaxis, hyperuricemia. Rare/Severe: Boxed warnings: Bleeding risk (can be fatal); reduced effectiveness when co-administered with maintenance aspirin doses > 100 mg daily; ventricular pauses, and intracranial hemorrhage.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black box warnings: (1) Major fatal bleeding; (2) Maintenance aspirin doses above 100 mg decrease the clinical efficacy of ticagrelor; co-administer ONLY with low-dose aspirin (75-100 mg). Contraindicated in active pathological bleeding and history of intracranial hemorrhage.
