Clinical Overview

Salbutamol (100mcg) and Ipratropium Bromide (20mcg Inhaler) combines a short-acting beta-2 agonist (SABA) with a short-acting muscarinic antagonist (SAMA) on the WHO Model List of Essential Medicines. Salbutamol relaxes bronchial smooth muscle via cyclic AMP elevation; ipratropium blocks vagal acetylcholine-mediated bronchoconstriction at muscarinic M3 receptors. This dual-pathway approach produces greater, longer-lasting bronchodilation than either agent alone without added sympathomimetic side effects. Indicated for management of bronchospasm in Chronic Obstructive Pulmonary Disease (COPD) and acute severe asthma exacerbations.

Usage Guidelines

Inhale 2 puffs four times daily through the MDI device (maximum 8 to 12 puffs daily). In acute exacerbations, take 2 puffs repeated every 20-30 minutes for up to 1 hour under clinical supervision. Use a spacer device for maximum lung delivery.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Dry mouth, fine skeletal muscle tremor (hands), palpitations, tachycardia, headache, nervousness, throat irritation. Rare/Severe: Paradoxical bronchospasm, acute urinary retention, narrow-angle glaucoma attack (if sprayed in eyes), and hypokalemia.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Avoid spraying into eyes (can precipitate acute angle-closure glaucoma). Use with caution in patients with prostatic hypertrophy, bladder neck obstruction, or ischemic heart disease. Not for long-term controller therapy in asthma (must use inhaled corticosteroids).
