Clinical Overview

Levodopa and Carbidopa is the gold-standard cornerstone treatment for Parkinson's disease on the WHO Model List of Essential Medicines. Levodopa crosses the blood-brain barrier via large neutral amino acid transporters and is decarboxylated by central DOPA decarboxylase into active dopamine in the striatum. Carbidopa does not penetrate the blood-brain barrier; it inhibits peripheral decarboxylation, preventing peripheral dopamine-mediated nausea and cardiovascular toxicity while increasing central levodopa availability tenfold. Indicated for Parkinson's disease and post-encephalitic parkinsonism.

Usage Guidelines

Take orally with water, preferably 30 to 60 minutes before meals or 1-2 hours after food. High-protein foods (meat, dairy, eggs) interfere with absorption; protein intake should be separated from dosing if motor fluctuations occur. Maintain strict timing of doses.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Nausea, anorexia, dyskinesias (choreoathetotic movements), dizziness, orthostatic hypotension, vivid dreams, insomnia. Rare/Severe: Hallucinations, psychosis, impulse control disorders (gambling, hypersexuality), sudden sleep onset episodes, and depression.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Contraindicated with non-selective MAOIs and in patients with narrow-angle glaucoma or suspicious undiagnosed skin lesions (melanoma). Long-term therapy is associated with motor complications (wearing-off, on-off phenomenon, peak-dose dyskinesia). Taper gradually.
