Clinical Overview

Gemcitabine Injection (200mg, 1000mg) is a difluoronucleoside pyrimidine antimetabolite on the WHO Model List of Essential Medicines. It is intracellularly phosphorylated to gemcitabine diphosphate (which inhibits ribonucleotide reductase, depleting deoxynucleotide pools) and gemcitabine triphosphate (which incorporates into replicating DNA, adding one additional nucleotide before DNA polymerases stall completely, masked from proofreading repair enzymes). Indicated for advanced pancreatic adenocarcinoma, non-small cell lung cancer (NSCLC), metastatic breast cancer, and advanced ovarian and bladder carcinomas.

Usage Guidelines

Administer intravenously only over 30 minutes diluted in 0.9% NaCl. Infusion time must NOT exceed 60 minutes or be given more frequently than once weekly, as prolonged infusions increase the formation of toxic intracellular metabolites and dramatically escalate myelosuppressive and pulmonary toxicity.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Bone marrow suppression (neutropenia, thrombocytopenia), flu-like syndrome (fever, chills, myalgia within 6-12 hours of infusion), transient proteinuria/hematuria, transaminase elevations, nausea. Rare/Severe: Hemolytic Uremic Syndrome (HUS) / Thrombotic Microangiopathy (TMA), acute respiratory distress syndrome (ARDS), and capillary leak syndrome.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Black Box Warning: Prolonging infusion time beyond 60 minutes or dosing more often than once weekly increases severe toxicity. Monitor CBC with differential prior to each dose. Discontinue immediately if signs of microangiopathic hemolytic anemia or respiratory distress occur.
