Clinical Overview
Modafinil is an atypical wakefulness-promoting agent. Unlike classical amphetamines, it acts as a selective, low-affinity dopamine transporter (DAT) inhibitor while stimulating hypocretin/orexin, histamine, and alpha-1 adrenergic systems in the hypothalamus and tuberomammillary nucleus, promoting alertness without peripheral sympathomimetic overstimulation or rebound hypersomnolence. Indicated for excessive daytime sleepiness associated with narcolepsy, obstructive sleep apnea (OSA), and shift work sleep disorder (SWSD).
