Clinical Overview
Isoxsuprine Injection IP 5 mg is a beta-2 adrenergic receptor agonist and direct vascular/uterine smooth muscle relaxant. By stimulating beta-2 adrenoreceptors in the myometrium, it activates adenylate cyclase and increases intracellular cAMP, decreasing intracellular calcium and producing profound relaxation of uterine smooth muscle (tocolysis); it also produces peripheral vasodilation. Indicated for the management of threatened abortion, threatened uncomplicated premature labor (tocolytic therapy between 20-36 weeks gestation), and peripheral vascular diseases (Raynaud's phenomenon, arteriosclerosis obliterans).

Usage Guidelines
For Intramuscular (IM) injection or Slow Intravenous Infusion ONLY: For threatened premature labor: 10 mg IM administered every 3 hours until contractions cease, or administered as a continuous IV infusion diluted in 5% Dextrose (0.2 to 0.5 mg/min titrated against uterine contractions and maternal pulse rate), followed by oral maintenance. Administered under continuous maternal cardiovascular and fetal heart rate monitoring. Patient must remain in lateral recumbent position to prevent supine hypotension.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Maternal and fetal tachycardia, palpitations, hypotension, flushing, nausea, vomiting, dizziness, and tremor. Rare/Severe: Maternal pulmonary edema (especially when co-administered with corticosteroids), severe hypotension, cardiac arrhythmias, and hypokalemia.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Discontinue immediately if maternal tachycardia exceeds 130-140 bpm or if signs of pulmonary edema develop. Contraindicated in recent myocardial infarction, cardiac arrhythmias, maternal severe heart disease, thyrotoxicosis, maternal hemorrhage, and intrauterine fetal demise. Avoid rapid IV injection.
