Clinical Overview
Ifosfamide Injection (1gm) is an oxazaphosphorine alkylating agent related to cyclophosphamide on the WHO Model List of Essential Medicines. It requires hepatic CYP3A4 bioactivation to ifosforamide mustard, which crosslinks DNA strands, and produces substantial quantities of toxic acrolein and chloroacetaldehyde. Indicated in combination with other antineoplastics for testicular cancer, soft tissue and bone sarcomas (Ewing sarcoma, osteosarcoma), non-Hodgkin lymphoma, and small cell lung cancer.

Usage Guidelines
Administer by slow intravenous infusion over at least 30 to 60 minutes (or continuous 24-hour infusion). MANDATORY: Mesna (at least 60% of the ifosfamide dose in divided increments) and aggressive IV hydration MUST ALWAYS be co-administered with every dose of ifosfamide to prevent lethal hemorrhagic cystitis.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Alopecia, myelosuppression, nausea, vomiting, hematuria. Rare/Severe: Severe hemorrhagic cystitis, central nervous system neurotoxicity / encephalopathy (confusion, hallucinations, cerebellar ataxia, coma due to chloroacetaldehyde), and Fanconi syndrome (nephrotoxicity).
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Hemorrhagic cystitis; Mesna uroprotection is MANDATORY. Black Box Warning: Neurotoxicity/encephalopathy (chloroacetaldehyde crossed into CNS); methylene blue (50 mg IV) is used as an antidote. Black Box Warning: Severe myelosuppression.
