Clinical Overview
Diethylcarbamazine (DEC) is a synthetic piperazine derivative anthelmintic and the mainstay treatment for filariasis on the WHO Model List of Essential Medicines. It alters microfilarial surface membranes, making them vulnerable to phagocytosis by host immune cells, and paralyzes adult worms. Indicated for lymphatic filariasis (caused by Wuchereria bancrofti, Brugia malayi), tropical pulmonary eosinophilia (TPE), and visceral larva migrans.

Usage Guidelines
Take orally immediately after meals with plenty of water to minimize gastrointestinal discomfort. Often prescribed in graded escalating doses over several days to diminish hypersensitivity reactions from dying microfilariae. Follow physician's exact dosing schedule.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Nausea, vomiting, headache, dizziness, drowsiness. Rare/Severe: Severe allergic reactions (Mazzotti reaction: fever, tachycardia, lymphadenitis, cutaneous swelling, hypotension), encephalopathy in co-infection with Loa loa, and optic nerve damage in onchocerciasis.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Contraindicated in patients with onchocerciasis (river blindness) due to severe ocular inflammatory damage. Rule out Loa loa infection before starting DEC to prevent fatal encephalopathy. Antihistamines or corticosteroids may be co-administered to control allergic reactions to dying parasites.
