Clinical Overview
Sildenafil Tablets (50mg, 100mg) and Orally Disintegrating Strips (50mg, 100mg) is a selective phosphodiesterase type 5 (PDE5) inhibitor on the WHO Model List framework. It inhibits PDE5 in the corpus cavernosum of the penis, preventing degradation of cyclic guanosine monophosphate (cGMP). During sexual stimulation, local nitric oxide release elevates cGMP, causing smooth muscle relaxation and cavernous arterial vasodilation, resulting in penile erection. Indicated for erectile dysfunction (ED) in adult men and off-label adjunctive therapy for pulmonary arterial hypertension.

Usage Guidelines
Tablets: Take orally 50 mg (range 25-100 mg) approximately 60 minutes prior to sexual activity with water. Maximum frequency is once daily. High-fat meals delay absorption and onset. Disintegrating strips: Place on tongue and allow to dissolve without water. Sexual stimulation is mandatory for therapeutic effect.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Facial flushing, headache, nasal congestion, dyspepsia, visual color tinge (blue-tinted vision / cyanopsia due to PDE6 inhibition in retina), back pain. Rare/Severe: Severe hypotension, myocardial infarction, non-arteritic anterior ischemic optic neuropathy (NAION), sudden hearing loss, and priapism (>4 hours duration).
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black Box Warning: Concomitant use with nitrates (nitroglycerin, isosorbide dinitrate) or guanylate cyclase stimulators (riociguat) is strictly contraindicated due to potentially fatal profound hypotension. Caution in patients with severe cardiovascular disease, recent stroke, or MI. Seek immediate emergency care for erections lasting >4 hours to avoid irreversible cavernous fibrosis.
