🧴 Clinical Application
Take orally with water, divided twice daily (morning and evening), with or without meals. Swallow whole; do not chew or crush. Follow prescribed schedule.
⚠️ Side Effects
Common: Severe headache (especially at initiation), dizziness, nausea, vomiting, flushing, weakness. Rare/Severe: Severe, refractory ulceration of the gastrointestinal tract, skin, and mucosal tissues (anal, oral, perianal ulcers, and gastrointestinal perforation); severe hypotension.

Major safety warning: Can cause serious gastrointestinal and mucosal ulcerations (oral, peristomal, anal ulcers, diverticular perforation); discontinue immediately if persistent ulceration develops. Absolute contraindication with PDE-5 inhibitors (sildenafil, tadalafil). Contraindicated in cardiogenic shock, severe hypotension, and acute pulmonary edema.

By opening sarcolemmal K_ATP channels, it hyperpolarizes vascular smooth muscle membranes, dilating coronary arterioles and reducing cardiac afterload, while its nitrate moiety stimulates guanylate cyclase to dilate systemic veins and reduce preload. Also provides cardioprotective ischemic preconditioning. Indicated for the prevention and long-term treatment of chronic stable angina pectoris.
