Imipenem provides potent bactericidal action by inhibiting bacterial cell wall synthesis, while cilastatin inhibits renal dehydropeptidase-I (DHP-I) in the renal brush border, preventing renal metabolism of imipenem, blocking nephrotoxic metabolites, and maintaining high urinary drug concentrations. Indicated for severe polymicrobial infections, hospital-acquired pneumonia, sepsis, intra-abdominal infections, and complicated UTIs.
