🧴 Clinical Application
Take orally with food to reduce gastric irritation, with the largest portion taken at bedtime. Sustained-release formulations must be swallowed whole without chewing or crushing. Taper slowly when discontinuing.
⚠️ Side Effects
Common: Dry mouth, constipation, somnolence, dizziness, tremor, sweating, sexual dysfunction, weight gain. Rare/Severe: High seizure risk (dose-dependent), cardiac arrhythmias, paralytic ileus, hyperthermia, and suicidality.
🛡️ Safety Warnings
Boxed warning for suicidality. Highest seizure incidence among classical antidepressants; avoid high doses (> 250 mg/day) and use caution in patients with lowered seizure thresholds. Contraindicated with MAOIs and post-MI.
