Clinical Overview
Lorazepam is a high-potency, intermediate-acting 3-hydroxy benzodiazepine on the WHO Model List of Essential Medicines. It enhances GABA-mediated synaptic inhibition by binding to GABAA receptors. Because it undergoes direct hepatic glucuronidation to inactive metabolites without intermediate oxidative CYP450 steps, it is the benzodiazepine of choice in patients with hepatic impairment and the elderly. Indicated for short-term management of severe anxiety, insomnia associated with anxiety, acute status epilepticus, and pre-anesthetic sedation.
