Clinical Overview
Lamotrigine is a phenyltriazine antiepileptic and mood stabilizer on the WHO Model List of Essential Medicines. It use- and voltage-dependently blocks voltage-gated sodium channels, suppressing the sustained repetitive firing of neurons and inhibiting the pathological release of the excitatory neurotransmitter glutamate. Indicated for focal seizures, generalized tonic-clonic seizures, Lennox-Gastaut syndrome, and maintenance treatment of bipolar I disorder to delay mood episodes.

Usage Guidelines
Swallow whole with water, with or without meals. Strict adherence to slow weekly dose titration is mandatory to minimize life-threatening rash. Do not stop abruptly.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Dizziness, ataxia, headache, diplopia, blurred vision, nausea, somnolence, insomnia. Rare/Severe: Boxed warning: Severe, life-threatening cutaneous reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis, especially with rapid titration or concurrent valproate), DRESS syndrome, aseptic meningitis, and hemophagocytic lymphohistiocytosis (HLH).
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Black box warning: Serious skin rashes (SJS/TEN); discontinue immediately at the first sign of any rash unless clearly unrelated. Co-administration with valproate inhibits lamotrigine clearance (requires more than 50% dose reduction and slower titration). Oral contraceptives accelerate clearance, cutting lamotrigine levels by half.
