Clinical Overview
Insulin Glargine (100 IU/mL) is a recombinant DNA-origin human insulin analogue with an ultra-long, peakless 24-hour pharmacokinetic profile on the WHO Model List of Essential Medicines. It contains two extra arginine residues at the B-chain C-terminus, shifting its isoelectric point to pH 6.7; upon subcutaneous injection into physiological neutral pH (7.4), it precipitates into a stable microcrystalline depot that releases insulin monomer slowly and constantly over 24 hours, simulating basal physiological insulin secretion. Indicated for once-daily basal glycemic control in type 1 and type 2 diabetes mellitus.

Usage Guidelines
Administered strictly by subcutaneous injection once daily at the exact same hour each day (usually bedtime or morning) using disposable pre-filled pen or insulin syringe. Rotate injection sites daily within the abdomen, thigh, or upper arm. Never administer intravenously or intramuscularly. Never mix or dilute in the same syringe with any other insulin.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Hypoglycemia, injection site pain, mild lipodystrophy, weight gain, peripheral edema. Rare/Severe: Severe, life-threatening hypoglycemia (seizures, coma), anaphylaxis, and severe hypokalemia.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Never mix with any other insulin formulation. Never inject IV. Do not freeze; store unopened pens in refrigerator (2-8°C); in-use pens can be kept at room temperature (<30°C) for up to 28 days. Daily injection site rotation is mandatory to prevent lipohypertrophy.
