Clinical Overview

Octreotide Injection (50mcg, 100mcg immediate-release; 20mg, 30mg Long-Acting Release LAR depot) is a synthetic octapeptide analogue of natural somatostatin on the WHO Model List of Essential Medicines. It binds with high affinity to somatostatin receptor subtypes sst2 and sst5, potently inhibiting the pathological hypersecretion of growth hormone (GH), insulin-like growth factor-1 (IGF-1), serotonin, gastrin, VIP, and glucagon, and causing splanchnic vasoconstriction. Indicated for symptomatic control of carcinoid syndrome (flushing, severe diarrhea), VIPomas, acromegaly, emergency management of bleeding gastroesophageal varices, and prevention of postoperative pancreatic fistula.

Usage Guidelines

Immediate-release: Administer subcutaneously or intravenously (50 to 100 mcg two to three times daily). For acute variceal bleeding: IV bolus 50 mcg followed by continuous IV infusion of 50 mcg/hr for 2 to 5 days. Depot LAR: Administer by deep intragluteal intramuscular (IM) injection only, once every 4 weeks by a healthcare professional.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Injection site pain, diarrhea, abdominal cramps, nausea, flatulence, cholelithiasis (gallstones or biliary sludge in up to 50% with chronic therapy), transient hypo- or hyperglycemia. Rare/Severe: Bradycardia, conduction abnormalities, and acute pancreatitis.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Chronic therapy induces gallstones or biliary sludge due to inhibition of gallbladder contractility; perform periodic gallbladder ultrasound. Modulates glycemic hormones; close blood glucose monitoring required in diabetic patients. LAR depot is strictly for intragluteal IM injection.
