Clinical Overview
Imeglimin Hydrochloride (500 mg, 1000 mg) is a first-in-class oral tetrahydrotriazine derivative "glimin" antidiabetic agent. It targets mitochondrial bioenergetics across multiple organs: in the pancreas, it restores glucose-stimulated insulin secretion by increasing mitochondrial ATP and closing K_ATP channels while protecting beta cells against apoptosis; in the liver, it inhibits complex I of the respiratory chain, suppressing excessive gluconeogenesis; and in skeletal muscle, it enhances complex III activity, improving mitochondrial fatty acid oxidation and glucose uptake. Indicated for the treatment of type 2 diabetes mellitus as monotherapy or adjunct to other antidiabetic agents.
