Clinical Overview

Bortezomib Injection (2mg, 2.5mg, 3.5mg) is a dipeptidyl boronic acid reversible inhibitor of the 26S proteasome on the WHO Model List of Essential Medicines. It binds to the chymotrypsin-like catalytic site of the 20S core particle, preventing degradation of ubiquitinated proteins. This leads to toxic accumulation of intracellular polyubiquitinated proteins, stabilization of I-kappa-B-alpha, blockade of nuclear factor-kappa B (NF-kB) activation, and activation of the unfolded protein stress response, selectively killing multiple myeloma cells. Indicated for the treatment of adult patients with multiple myeloma and mantle cell lymphoma.

Usage Guidelines

Administer by SUBCUTANEOUS (SC) injection into the thighs or abdomen (strongly preferred over intravenous injection due to dramatically lower incidence of peripheral neuropathy). Can also be administered by rapid 3 to 5 second intravenous bolus. Never administer intrathecally. Standard cycles administer on days 1, 4, 8, and 11 of a 21-day cycle, maintaining at least 72 hours between consecutive doses. MANDATORY: Antiviral prophylaxis (e.g. acyclovir 400 mg twice daily) is required to prevent herpes zoster reactivation.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Peripheral sensory neuropathy (numbness, neuropathic pain in feet/hands), fatigue, diarrhea, nausea, thrombocytopenia (cyclical nadir at day 11 with rapid recovery), constipation. Rare/Severe: Severe autonomic neuropathy, posterior reversible encephalopathy syndrome (PRES), congestive heart failure, acute lung injury, and herpes zoster reactivation.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Subcutaneous administration is strongly recommended over IV to reduce severe peripheral neuropathy by up to 50%. Mandatory concomitant antiviral prophylaxis against shingles (Herpes zoster). Fatal if administered intrathecally. Substrate of CYP3A4.
