Clinical Overview
Bempedoic Acid is an oral, first-in-class non-statin adenosine triphosphate-citrate lyase (ACL) inhibitor. It acts upstream of HMG-CoA reductase in the hepatic cholesterol biosynthesis pathway. Because it is a prodrug requiring activation by very long-chain acyl-CoA synthetase-1 (ACSVL1), an enzyme present exclusively in the liver and absent in skeletal muscle, it inhibits cholesterol synthesis without causing the myopathy and muscle pain common to statins. Indicated for primary hyperlipidemia and to reduce cardiovascular risk in patients with atherosclerotic cardiovascular disease who are statin-intolerant.

Usage Guidelines
Take orally once daily with water, with or without food, at the same time each day. Can be used alone or in combination with ezetimibe. Maintain dietary therapy.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Hyperuricemia (elevated blood uric acid), gout flares, abdominal pain, back pain, anemia, muscle spasms. Rare/Severe: Tendon rupture (Achilles tendon, biceps, rotator cuff), severe hyperuricemia, and elevated liver enzymes.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Increases serum uric acid levels; monitor for gout, especially in patients with prior gout history. Increased risk of tendon rupture; discontinue immediately if tendon pain or swelling occurs. Avoid co-administration with simvastatin doses > 20 mg or pravastatin > 40 mg.
