Clinical Overview

Metolazone is a quinazoline-derivative thiazide-like diuretic. It inhibits sodium reabsorption at the cortical diluting segment of the distal renal tubule and proximal tubule. Uniquely, it retains its diuretic potency even in advanced renal failure (GFR < 20 mL/min) where standard thiazides fail, and exhibits profound synergistic diuresis ("sequential nephron blockade") when combined with loop diuretics. Indicated for severe refractory edema accompanying congestive heart failure and renal disease, and hypertension.

Usage Guidelines

Take orally once daily in the morning with water, with or after breakfast. When co-administered with loop diuretics (furosemide/torasemide), take 30 minutes prior to the loop diuretic. Rigorous electrolyte monitoring is essential.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Hypokalemia, hyponatremia, hyperuricemia, dizziness, orthostatic hypotension, dehydration. Rare/Severe: Profound, life-threatening electrolyte depletion, circulatory collapse, acute kidney injury, and necrotizing pancreatitis.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Extreme potency when combined with loop diuretics can provoke massive diuresis, acute hypovolemia, and dangerous hypokalemia within 24-48 hours; frequent monitoring of serum electrolytes and renal function is mandatory. Contraindicated in anuria and hepatic coma.
