Clinical Overview
Oxcarbazepine is a 10-keto structural analogue of carbamazepine. It is rapidly and completely reduced by cytosolic arylketone reductase into its active 10-monohydroxy metabolite (MHD; licarbazepine), which blocks voltage-sensitive sodium channels and high-voltage-activated calcium channels. Unlike carbamazepine, it does not form a toxic epoxide intermediate and does not undergo hepatic auto-induction. Indicated as monotherapy or adjunctive therapy for partial-onset seizures with or without secondary generalization.

Usage Guidelines
Take orally with water, with or without food, twice daily at 12-hour intervals. Swallow whole. Discontinue only by gradual dose reduction over several weeks.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Dizziness, somnolence, diplopia, fatigue, nausea, vomiting, ataxia, clinically significant hyponatremia. Rare/Severe: Severe hyponatremia (SIADH, serum sodium < 125 mmol/L), Stevens-Johnson syndrome, anaphylaxis/angioedema, and multiorgan hypersensitivity.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
High risk of clinically significant hyponatremia; baseline and regular serum sodium monitoring is recommended, particularly in elderly patients or those taking diuretics. Cross-hypersensitivity with carbamazepine occurs in 25-30% of patients. Hormonal contraceptives are rendered less effective.
