Recombinant Human Insulin formulation (Regular human insulin, Isophane NPH intermediate-acting insulin, or Biphasic premixed human/analogue insulin) on the WHO Model List of Essential Medicines. It replaces deficient endogenous insulin, binding to membrane insulin receptor tyrosine kinase to promote cellular glucose uptake into skeletal muscle and adipose tissue, stimulate glycogen and protein synthesis, and inhibit lipolysis and hepatic gluconeogenesis. Indicated for type 1 diabetes mellitus, type 2 diabetes mellitus uncontrolled on oral therapies, gestational diabetes, and diabetic ketoacidosis.

Usage Guidelines
Administered by subcutaneous injection 15 to 30 minutes before meals (for regular/premixed) or immediately before meals (for rapid analogues). Rotate injection sites systematically across the abdomen, thighs, buttocks, and upper arms. Gently invert cloudy NPH/biphasic suspensions 10-20 times before each dose to resuspend uniformly; do not shake vigorously.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Hypoglycemia (tremor, diaphoresis, palpitations, confusion), localized lipodystrophy, injection site redness, weight gain. Rare/Severe: Severe hypoglycemic coma, severe hypokalemia, and systemic hypersensitivity.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
High-alert medication; verify dose, units, and syringe scale (U-40 vs U-100) carefully before every injection. Always carry rapid-acting oral glucose or candy. Do not freeze. Unopened vials refrigerated (2-8°C); in-use vials room temperature (<25-30°C) for 28-42 days. Rotate injection sites to prevent lipohypertrophy.
