Clinical Overview
Natamycin (5% w/v ophthalmic suspension) is a tetraene polyene broad-spectrum fungicidal antibiotic on the WHO Model List of Essential Medicines. It is the premier drug of choice for fungal keratitis. It binds selectively to fungal cell membrane ergosterol, disrupting membrane permeability and causing lethal leakage of essential ions and macromolecules. Highly active against filamentous fungi (Fusarium, Aspergillus, Curvularia) and yeasts (Candida). Indicated for fungal blepharitis, conjunctivitis, and fungal keratitis.

Usage Guidelines
Shake the bottle vigorously before every dose. For fungal keratitis: instill one drop into the affected eye every 1 to 2 hours around the clock for the first 3 to 4 days, then reduce to 1 drop 6 to 8 times daily. Treatment must continue for a minimum of 2 to 3 weeks until complete resolution.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Eye irritation, foreign body sensation, punctate keratitis, visual blurring, conjunctival chemosis. Rare/Severe: Corneal opacification, allergic blepharitis, and eye pain.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Keep suspension in the refrigerator; do not freeze. Inadequate treatment duration can lead to fungal recurrence. Contact lenses are strictly forbidden during active keratitis. Regular slit-lamp monitoring is essential.
