Clinical Overview

Cilnidipine is a novel fourth-generation calcium channel blocker with unique dual L-type and N-type voltage-gated calcium channel antagonist properties. While L-type blockade dilates vascular smooth muscle, N-type blockade at sympathetic nerve terminals inhibits the pathological release of norepinephrine, suppressing sympathetic nervous hyperactivity, preventing reflex tachycardia, and dilating both afferent and efferent renal arterioles to lower intraglomerular hydrostatic pressure. Indicated for essential hypertension, particularly in patients with morning hypertension, diabetic nephropathy, chronic kidney disease, or ankle edema from amlodipine.

Usage Guidelines

Take orally once daily after breakfast or with water, at the same time each day. Swallow whole without crushing or chewing.

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Common: Dizziness, flushing, headache, mild palpitations, nausea, rash. Rare/Severe: Severe hypotension, excessive bradycardia, and elevation of liver enzymes.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Contraindicated in patients with severe cardiogenic shock or advanced aortic stenosis. Safe alternative in patients experiencing refractory pedal edema from amlodipine. Caution in severe hepatic impairment.
