Clinical Overview
Pregabalin and Nortriptyline combines a gabapentinoid voltage-gated calcium channel modulator with a tricyclic antidepressant (TCA). Pregabalin binds the alpha-2-delta auxiliary subunit of presynaptic voltage-gated calcium channels in the CNS, diminishing calcium influx and the exocytosis of excitatory neurotransmitters (glutamate, substance P, noradrenaline). Nortriptyline inhibits presynaptic reuptake of serotonin and noradrenaline, enhancing descending inhibitory pain pathways in the spinal dorsal horn. Indicated for moderate-to-severe neuropathic pain, diabetic peripheral neuropathy, and post-herpetic neuralgia.

Usage Guidelines
Take orally once daily at bedtime with a glass of water, with or without food. Bedtime dosing helps manage somnolence and anticholinergic drowsiness. Do not discontinue abruptly; taper gradually under medical direction.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Somnolence, dry mouth, dizziness, weight gain, blurred vision, constipation, urinary hesitancy. Rare/Severe: Suicidal ideation, cardiac arrhythmias, orthostatic hypotension, angioedema, and serotonin syndrome.
*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.
Precautions & Warnings
Contraindicated during acute recovery phase following myocardial infarction and concomitant use of MAO inhibitors. Caution in elderly patients, cardiac conduction defects, narrow-angle glaucoma, and urinary retention. Avoid alcohol consumption.
