Clinical Overview

Triamcinolone (Acetonide Injection IP 40mg/ml) is an intermediate-acting synthetic fluorinated corticosteroid with 5 times the anti-inflammatory potency of hydrocortisone and no mineralocorticoid activity, on the WHO Model List of Essential Medicines. Formulated as a microcrystalline suspension for depot injection, it provides sustained local anti-inflammatory effect over 3 to 6 weeks. Indicated for intra-articular injection in severe osteoarthritis/rheumatoid arthritis, intralesional injection in keloids/alopecia areata, deep IM systemic therapy for chronic severe allergic conditions, and oral therapy for inflammatory disorders.

Usage Guidelines

For Intra-articular, Intralesional, or Deep IM injection ONLY: NEVER administer intravenously. Shake vial well to ensure uniform suspension. For intra-articular: 5 mg to 40 mg into the joint space under strict aseptic technique. For deep IM: 40 mg to 80 mg injected deep into the gluteal muscle (avoid subcutaneous fat to prevent fat atrophy and skin depigmentation).

General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.

Possible Side Effects

Injection: Local injection site cutaneous/subcutaneous fat atrophy, skin depigmentation, post-injection flare, and tendon rupture. Systemic: Dyspepsia, insomnia, hyperglycemia, fluid retention, and adrenal suppression.

*Note: Individual physiological responses vary. While many patients experience few or no side effects, notify your treating physician if adverse reactions persist or if hypersensitivity (angioedema, skin rash, dyspnea) occurs.

Precautions & Warnings

Black Box Warning: Strict contraindication against IV administration (microcrystalline formulation causes pulmonary embolism). Contraindicated in idiopathic thrombocytopenic purpura (IM route), systemic fungal infections, and septic arthritis.
