Clinical Overview
Permethrin (5% w/w cream/lotion or 1% soap) is a synthetic pyrethroid neurotoxic pediculicide and scabicide on the WHO Model List of Essential Medicines. It binds to voltage-gated sodium channels in the nerve membranes of arthropods (Sarcoptes scabiei mites, Pediculus humanus lice), delaying channel repolarization, causing persistent sodium influx, paralysis, and death of the parasites and their ova. Indicated for the treatment of scabies infestation and head/pubic lice.

Usage Guidelines
For scabies: Apply cream/lotion thoroughly to the entire body from the neck down to the soles of the feet, paying special attention to interdigital spaces, wrists, axillae, umbilicus, genitalia, and under fingernails. Leave on for 8 to 14 hours (usually overnight) before washing off completely with warm water. All close household contacts must be treated simultaneously. Wash bedding and clothing in hot water. Repeat once after 7 days if active mites persist.
General Administration Advice: Adhere strictly to the prescribed dosage, frequency, and treatment course. For oral medications, swallow with sufficient water. Never stop antibiotics, psychiatric medications, or antihypertensives abruptly without clinical direction.
Possible Side Effects
Common: Transient burning, stinging, numbness/tingling (paresthesia), mild erythema, pruritus (scabies itch often persists for 2-4 weeks after mites are killed due to allergic reaction to dead mites). Rare/Severe: Severe allergic contact dermatitis.

For external use only. Avoid contact with eyes and mucous membranes. Treat all household contacts at the same time to prevent re-infestation. Post-scabietic itch is common and does not indicate treatment failure.
