Nalidixic Acid

🧴 Clinical Application

Take orally 1 g (two 500 mg tablets) four times daily with a glass of water, with or after meals for 7 to 14 days. Reduce to 500 mg four times daily for prolonged therapy. Maintain adequate fluid intake.

⚠️ Side Effects

Common: Nausea, vomiting, abdominal pain, drowsiness, dizziness, photosensitivity reactions. Rare/Severe: Intracranial hypertension (pseudotumor cerebri, bulging fontanelles in infants), seizures, psychosis, and visual disturbances (colors appear halos).

Contraindicated in infants under 3 months, history of convulsive disorders, and porphyria. Avoid strong sun exposure due to photosensitivity. Complete the entire course; resistance can emerge rapidly if underdosed.

It selectively inhibits bacterial DNA gyrase (topoisomerase II), blocking bacterial DNA replication and transcription in gram-negative uropathogens including E. coli, Proteus, Klebsiella, and Enterobacter. Indicated for the treatment of acute and chronic uncomplicated urinary tract infections.
