🧴 Clinical Application

Take orally once daily (or twice daily for bosentan) with a glass of water, with or without food. Swallow tablets whole. Strict monthly liver function tests and pregnancy tests are mandatory.

Common: Peripheral edema, fluid retention, nasal congestion, headache, flushing, anemia. Rare/Severe: Boxed warnings: Major birth defects/fetal toxicity; severe hepatotoxicity/liver failure (especially with bosentan), and severe decrease in hemoglobin/hematocrit.

Endothelin Receptor Antagonist (ERA) on the WHO Model List of Essential Medicines. Endothelin-1 is a potent endogenous vasoconstrictor and vascular smooth muscle mitogen; ERAs selectively or dualistically block endothelin receptors (ETA and ETB) on pulmonary vascular smooth muscle, reversing pulmonary vasoconstriction, inhibiting vascular remodeling, and reducing pulmonary vascular resistance. Indicated for Pulmonary Arterial Hypertension (PAH, WHO Group 1) to improve exercise capacity and delay clinical worsening.
